Participation Waiver

ACKNOWLEDGEMENT OF RISK AND AGREEMENT:
I consent to my participation in this program. I understand that attending this program involves some risks, including the possibility of damage to my property or serious injury to me. I agree, on behalf of myself and my, to assume all the risks in connection with my participation in the program. I agree that I will follow relevant Let's Grow Sports directions, rules, and policies and will obey directions. I acknowledge that Let's Grow Sports has the right to remove me from participation in the program if they are not complying with the rules and regulations of the program.

HEALTH AND SAFETY:
I know of no health-related reasons or problems, which preclude or restrict me from participating in the program.

MEDICAL RELEASE: 
I grant medical personnel permission to provide medical care for conditions, which may arise during participation at this program. Let's Grow Sports does not assume any responsibility nor provide any insurance for medical expenses incurred by your dependents as a result of any accident or sickness while they are participating in this program.

RELEASE OF LIABILITY:
I agree to release Let's Grow Sports, its trustees, officers, employees, volunteers, members, and representatives from any and all liabilities and claims whatsoever arising out of, or in connection with, my attendance and participation in the program, even those that may have been caused by the ordinary negligence of Let's Grow Sports and the facility staff and affiliates.

MEDIA RELEASE:
Let's Grow Sports may make and use images, film, video, and audio recordings of me, and take and use quotes or statements from me, during the program, without compensation. I release all claims against Let's Grow Sports with respect to privacy, copyright ownership, and publication, related to the use of the quotes, images, or recordings.

ACKNOWLEDGEMENT:
By reading and signing this legally binding document, I know I am voluntarily waiving the right to sue Let's Grow Sports if I am injured while participating in this program.

COVID-19 RELEASE OF LIABILITY AGREEMENT

Assumption of Risk. I hereby acknowledge that the Coronavirus (COVID-19) outbreak has been classified by the World Health Organization as a pandemic, and that COVID-19 is highly contagious and may result in serious personal injury or death if contracted. I understand that, while Let's Grow Sports, has undertaken steps to reduce the risk of transmission of COVID-19 in connection with its programs and events, Let's Grow is not responsible for any risks related to COVID-19 in connection with its programs and events. I acknowledge that participation in Let's Grow programs and events involves certain inherent risks related to COVID-19 transmission that cannot be eliminated regardless of care taken to avoid such risks, and I hereby voluntarily accept and assume all risk of loss, personal injury, sickness, death, damage, and expense arising from such risks. I understand that my voluntary participation in Let's Grow programs and events increases my risk of exposure to and infection by COVID-19, and acknowledge that I must comply with all procedures set by Let's Grow to reduce the spread of COVID-19 while participating in Let's Grow programs and events.

Further, I hereby attest that: (1) I am not experiencing any symptoms of illness, including but not limited to cough, difficulty breathing, fever, chills, muscle pain, headache, sore throat, or new loss of taste/smell; (2) I have not traveled to a high risk area as determined by the Centers for Disease Control and Prevention (CDC) in the last 14 days; (3) I do not believe I have been exposed to someone with a suspected or confirmed case of COVID-19; and (4) I am following all CDC guidelines as much as possible to limit my exposure to COVID-19. 

Release and Waiver. I hereby release, waive, discharge, and covenant not to sue Let's Grow and its past, current, and future directors, employees, members, volunteers, representatives, affiliates, and assigns from any and all liability, claims, actions, litigation, demands, damages, injuries, or losses, including but not limited to those for personal injury, sickness, or death, which may be incurred, directly or indirectly, now or in the future, in any way related to COVID-19 and in connection with my participation in Let's Grow programs and events. This Agreement shall be binding on my heirs, executors, administrators, successors, and assigns. The invalidity of any part of this Agreement shall not affect the validity of the remaining parts of this Agreement. I expressly agree that this Agreement is intended to be as broad and inclusive as is permitted by applicable laws, and will remain in effect until laws and mandates relevant to COVID-19 are lifted.
